Consent Form:

Body sculpting increases flow of both the lymphatic and circulatory systems, and it also helps with cleaning of the tissues. The main use of body sculpting treatments is inch loss, diminishing of cellulite, and tightening of the skin. Benefits: Lose 1-3 inches per treatment with state -of-the-art equipment. Benefits are often immediate but may be delayed in some people.

For Best Results: Series of 6-12 treatments/ area (sometimes more) with 3 days between each treatment. This is not a weight loss treatment. You must maintain your results by nutrition and proper exercise and drinking 2 liters of water per day are always recommended. It is recommended that you exercise within 4-6 hours of treatment and avoid sugar and alcohol for 48-72 hours after very treatment.

Precautions: Body sculpting treatments are not recommended if you are pregnant, breast feeding, have a lymphatic disorder, acute illness, metal implants, pacemakers, or are currently being treated for active cancer.

Waiver: I understand that I am using 5 in 1, 40k slim machine, vacuum cupping machine provided by Wild Beauty Bar at my own risk. Should I sustain an injury while using the equipment, I agree to not hold Wild Beauty Bar responsible.

Acknowledgement: I read, understand & acknowledge contents of this consent form for Wild Beauty Bar. Prepayments are non-refundable (includes packages). I will give more than 24 Hour to cancel/reschedule an appointment, to avoid the Cancellation/No-Show charge (full price of appointment).

I consent to allow Wild Beauty Bar staff members to consult with & evaluate me in order to determine if I am a good candidate for the non-surgical body sculpting/contouring service(s). I understand that photographs and measurements will be taken and kept in my file. I agree that these forms have been completed truthfully and to the best of my knowledge/abilities.

I authorize Wild Beauty Bar Photo & Video Consent of the following:

to copyright, use and publish and of the images in any format taken of me

images may be used for a variety of purposes and may appear on the website, promotional materials or any other media now known or to be invented

Wild Beauty Bar will use the images exclusively for Wild Beauty Bar related purposes and not for any commercial gain.

Since anyone can download an image from the internet or make copies from printed materials, I agree that for Wild Beauty Bar is not responsible for unauthorized use of images. I am aware that I am not entitled to any compensation and that the images may appear without my name.

I knowledge & understand the Wild Beauty Bar Cancellation Policy:

If there is a need to cancel for any reason, it must be done more than 24 hours before your booked appointment time. Please understand that when you do not cancelor show up for an appointment it is a cost to us. If you do not provide us with more than 24 hours notice, you will be charged the Cancellation/ No-Show fee, which is the full price of the appointment booked.

If you had made a deposit for your appointment then your deposit will be forfeited to Wild Beauty Bar.
